As of Sunday, December 18, 2022, Jefferson Surgical Clinic (JSC) physicians and staff are employed by Carilion Clinic.

JSC and Carilion share the same philosophy about the practice of medicine. As a patient, you’ll receive the same care and compassion from your surgical provider. Also, you’ll have access to additional resources including MyChart, a secure online health management tool, and easy connections to many specialty services through an electronic medical record system.

The patient portal is no longer active due to system changes. You are able to pay your account balance using the “Bill Pay” button below instead of calling. For account questions, you may still call (540) 283-6070.

New Patients

Welcome to Jefferson Surgical Clinic! New patients will find a link to each of our physician’s new patient packets. Each packet includes forms, information about our offices and driving directions. If you would like to complete all your paperwork before your visit, click on the link, complete the forms and bring them with you to your appointment.

Each time you visit our offices, please be sure to bring:

  • Current insurance cards
  • A list of all medications you currently take. Include medication name, dosage, and name of the prescribing physician
  • Co-payment, as required by insurance

In case of an after-hours emergency, please call Carilion Roanoke Memorial Hospital at (540) 981-7000 and ask for the physician on call.

If you are a patient at the LewisGale Medical Center in Salem and have an after-hours emergency, please call (540) 776-4000 and ask for the physician on call.

Calls for new prescriptions should be made during our normal office hours.

If you need to refill your current prescription, call your pharmacy and give them the information from the label on your bottle. The pharmacy will contact us.

Please anticipate your prescription needs as it may take 24 to 48 hours for the doctor to return a pharmacist’s call.

Narcotics are not prescribed or renewed over the phone or after hours.

If we have not seen you in our office within the past year, we will not authorize new prescriptions or prescription refills. You will need to make an appointment to be seen by your physician.

You can make payments to Jefferson Surgical Clinic using cash, check, money order, Visa, MasterCard or Discover. We also participate in Project Access.

Insurance co-pays are expected to be paid each time you visit Jefferson Surgical Clinic.

If you are a self-pay patient, please be prepared to pay at least $200 toward your medical care at the time of your appointment (unless prior arrangements have been made with our billing department).

Accounts that are not paid within 60 days are considered past due (unless special arrangements have been made with our billing department).

Current insurance or enrollment cards must be presented at the time of registration for each visit. Please be sure to bring insurance cards with you.

Please advise our staff of any changes in your insurance coverage, employment or address information when you register for your appointment.

When all of your insurance information has been received, we will automatically file all insurance claim forms for you.

You will be required to authorize payment of insurance benefits directly to our office unless you pay your balance in full at the time of your visit.

Our professional staff of billing and coding specialists is available to assist you with billing, insurance and payment questions. We can work with you to establish payment arrangements and determine insurance coverage or eligibility for financial assistance. If your insurance requires that you have a referral from your primary care physician to see one of our physicians, it is your responsibility to obtain that referral and have it sent to our office before your appointment. You cannot be seen if a referral is needed and is not obtained prior to the appointment.

  • Provide you with this notice as to our legal duties and privacy practices with respect to information we collect and maintain about you.
  • Abide by the terms of this notice.
  • Notify you if we are unable to agree to a requested restriction.
  • Accommodate reasonable requests you may have to communicate health information by alternative means or at alternative locations.
